The Immunohaematology Working Party aims to promote best practice and drive improvements in the field through education, the exchange of ideas and resources, and support for red cell serology, molecular testing, antigen and allele matching, and transfusion recommendations.

Membership

The Working Party has 42 members representing 24 countries across six continents: North America (Canada, United States) with 8 members; South America (Brazil) with 3; Europe (Croatia, Finland, France, Germany, Italy, Netherlands, Spain, Switzerland, UK) with 14; Africa (South Africa) with 1; Asia (China, India, Iran, Malaysia, Oman, Qatar, Saudi Arabia, Taiwan, UAE) with 13; and Oceania (Australia, New Zealand) with 3.


Meetings and Teleconferences

During the reporting period, several virtual and in-person meetings were held to progress Working Party (WP) activities and strategic priorities.

A virtual meeting between the WP Chair and WP Board Liaison took place on 11 April 2025 to discuss WP matters requiring Board Liaison support. This was followed by an Executive Committee meeting on 9 May 2025, focusing on new member applications and planning for the Milan ISBT Congress 2025. A further Executive Committee meeting was held on 20 May 2025 to finalise preparations for the WP Business Meeting during the Milan Congress.

The WP Business Meeting was held in person on 31 May 2025 during the Milan Regional ISBT Congress. A total of 19 members attended in person and 16 joined via videoconference, while 7 members were absent. In addition, 8 guests and 7 Corporate Partners attended. Prior to the meeting, members were asked to identify their top immunohaematology laboratory challenge, which informed discussions on key focus areas for WP subgroups. The main themes included:

  • Serological Testing: challenges in assessing clinical significance of alloantibodies to high-prevalence antigens, limited blood volume testing methods, need for improved exchange of rare blood samples and antisera, compatibility testing in patients treated with anti-CD38/CD47 therapies, low-affinity antibodies, and platforms for sharing rare or unidentified antibody cases.
  • Molecular Testing: resolution of ambiguous allele results in commercial assays, sharing of rare alleles, defining combined serologic/molecular testing algorithms, expanding use of genotyping, and improving patient versus donor screening approaches.
  • Education and Training: need for enhanced training in new serological and molecular technologies and addressing workforce shortages in immunohaematology expertise.
  • Immunohaematology Reagents: issues related to cost recovery for laboratory analysis and difficulties in international sharing of rare reagents.
  • Quality Assurance & Accreditation: need for IT solutions for secure data exchange, and challenges related to QC requirements where commercial controls are limited and regulatory understanding is insufficient.

An in-person Executive Committee meeting was held on 15–16 December 2025 at the ISBT Central Office in Amsterdam. During this meeting, subgroup tasks were defined based on priorities identified at the Milan Business Meeting. A key issue identified was the shortage of rare cells and antisera for confirming genetic findings and achieving case resolution. This will be addressed by the Reagents Subgroup in collaboration with the Rare Donors Working Party through the development of a sample exchange programme. The Committee also explored the development of both wet and dry workshops, including a mobile “toolbox” model and in-situ training visits by immunohaematology experts, led by the Training and Education Subgroup. Additional discussions included review of case studies submitted for publication, planning of future Executive Committee meetings (including four online meetings and one joint meeting with subgroup leaders), and a meeting with the Board Liaison and Special Advisor regarding future WP direction.

A further virtual Executive Committee meeting was held on 20 January 2026 to follow up on actions from the December meeting, including finalisation of WP funding requests for 2026/27 and progression of voting for new members.


Activities during the period

  • WP Chair was invited as IH WP representative to contribute to the program planning of the ISBT and Thalassemia International Federation (TIF) “Together for Thalassaemia Care” Webinar series, to coincide with 2025 International Thalassemia Day on 8th May. The series consisted of two 1.5hr webinars, running on consecutive days on 7th and 8th May. WP Chair presented on the second day, presentation titled “Avoiding Alloimmunisation and Providing Compatible Blood”. The program included patients which helped to give a more complete representation of the impact of thalassemia and the benefits of optimal treatment.
  • Our first ever IH WP Training Travel Bursary recipient travelled from Cameroon to complete their 2-week training placement at the International Blood Group Reference Laboratory (IBGRL), NHSBT in Bristol, UK 19 – 30MAY2025. Our well-deserved recipient provided a lovely letter of appreciation and provided overwhelmingly positive feedback. The team at IBGRL also found the whole experience very rewarding.
  • Interactive Workshop: “Tackling Complex Cases in the Immunohaematology Lab” including interactive case studies and quiz was organised and took place at the ISBT Milan Congress 2025. Feedback was overwhelmingly positive and defined a new session designation of “edutainment”.
  • WP Treasurer presented as an IH WP representative invited speaker at the Ain Shams University Hospitals (ASUH) Workshop on Immunohaematology in Transfusion Medicine: Fundamental Concepts and Real-world Applications, which took place at ASUH in Cairo, Egypt, 20 – 21OCT 2025. Two presentations were prepared and delivered, “Special Laboratory Techniques to Crack Tough Antibody Cases” and “Tackling Complex Cases in the Immunohaematology Reference laboratory – Interactive”.
  • Our suggestion for the Kuala Lumpur 2026 Congress program, “Interactive Immunohaematology Case Studies & Blood Group Bingo” was accepted. Planning for this session started during this reporting period.

Challenges and Opportunities

  • One particular challenge encountered regarding the IH WP Training Travel Bursary was regarding long processing times for the necessary visas. A lesson we have learned is that notification of the bursary award should be well in advance of the intended travel dates to enable timely planning for travel documents.
  • Another challenge our WP faces (though it is a nice problem to have) is the significant interest and many applications for WP membership. We need to develop more robust guidelines/rules for membership to ensure the administration of membership is less of a burden, whilst maintaining the integrity and expertise of our membership, in parallel with providing development opportunities.

Summary

The ISBT Working Party on Immunohaematology has seen another strong year of acheivements, the highlight being the successful completion of the first IH Training Travel Bursary 2-week training placement which was a huge success. Other key outcomes during this period have been the identification of priority focus areas for the WP subgroups which will provide clear direction for acheiving tangible outputs, and organisation, participation and engagement in several interactive IH themed educational events. Future priorities will focus on subgroup led action plans to acheive our ambitious plans to tackle the most significant challenges facing the Immunohaematology Laboratory today. We will continue to prioritse education and training in IH and offer development opportunities through mentorship and training programs.
